We are seeking a professional, dynamic, and caring Hospital Social Worker to join our team! The ideal candidate will have a strong background in health and social care, preferably within a medical or hospital setting. The role's key focus is to provide education to families and individuals about illnesses and treatment plans.

Responsibilities

• Educate patients and families regarding illnesses and potential treatment plans
• Conduct psychosocial assessments to identify the presence of mental or emotional distress
• Provide counseling to those facing crisis or experiencing distress
• Connect patients and their families to necessary resources and service providers.
• Detailing reports on patients to other healthcare professionals or service providers.
• Maintaining detailed case files on each patient.
• Work in line with HIPPA guidelines and follow safeguarding procedures.
• Always keep patient records up to date in the database.
• Attend professional workshops and conferences to maintain up-to-date skills and knowledge of industry best practices.

Requirements

• A minimum of a Bachelor's degree in Social work, Psychology or related field
• Minimum of 2 years of experience in the social work industry, preferably within a hospital or healthcare setting
• Knowledge of HIPPA guidelines and safeguarding
• Compassionate and kind in nature
• Good computer skills, including Microsoft office and record-keeping software
• Excellent communication skills, both written and verbal
• Highly organized
• Excellent interpersonal skills
• Good problem-solving skills
• Excellent note-taking abilities
