We are seeking a professional, dynamic, and caring Hospital Social Worker to join our team! The ideal candidate will have a strong background in health and social care, preferably within a medical or hospital setting. The role's key focus is to provide education to families and individuals about illnesses and treatment plans.
Responsibilities
- Educate patients and families regarding illnesses and potential treatment plans
- Conduct psychosocial assessments to identify the presence of mental or emotional distress
- Provide counseling to those facing crisis or experiencing distress
- Connect patients and their families to necessary resources and service providers.
- Detailing reports on patients to other healthcare professionals or service providers.
- Maintaining detailed case files on each patient.
- Work in line with HIPPA guidelines and follow safeguarding procedures.
- Always keep patient records up to date in the database.
- Attend professional workshops and conferences to maintain up-to-date skills and knowledge of industry best practices.
Requirements
- A minimum of a Bachelor's degree in Social work, Psychology or related field
- Minimum of 2 years of experience in the social work industry, preferably within a hospital or healthcare setting
- Knowledge of HIPPA guidelines and safeguarding
- Compassionate and kind in nature
- Good computer skills, including Microsoft office and record-keeping software
- Excellent communication skills, both written and verbal
- Highly organized
- Excellent interpersonal skills
- Good problem-solving skills
- Excellent note-taking abilities
