We are seeking a professional, personable, and dynamic Family Nurse Practitioner to join our team! The ideal candidate for the role will have a wealth of experience in providing diagnosis, advice, and wellness checks, along with medication administration to families from various backgrounds and diversities. The role's key focus will be to support families in leading a healthy lifestyle, ensuring that required medication and treatments are provided, and ensuring accurate assessments and diagnoses are completed.

Responsibilities

• Consult with families to understand their dynamics and any health concerns they have.
• Assess and diagnose issues or illnesses, or complaints raised by any family member.
• Treat, prescribe and administer medication as needed.
• Order investigative and diagnostic tests as needed.
• Teach and mentor patients on living a healthy and balanced lifestyle
• Address issues within the family concerning obesity or hereditary conditions.
• Promote wellness and help to prevent diseases or conditions from arising that are preventable.
• Conduct regular physical assessments.
• Create and develop treatment plans for families/patients
• Keep records and case files for each family or patient.
• Make referrals where required to other healthcare service providers.
• Work in line with safeguarding and patient confidentiality best practices

Requirements

• Bachelor’s degree in nursing or related field
• License to practice as a nurse in the country/state you reside
• Minimum of 2 years of hands-on experience in a nursing role
• Demonstrated experience in collecting, analyzing and evaluating data and information
• Excellent communication skills and relationship-building abilities
• Strong knowledge of patient confidentiality and safeguarding practices
• Well-developed interpersonal skills
• Highly detail focused
• Able to create and execute care plans
• Good problem-solving skills
• Kind, patient and caring in nature
