Job brief
We are seeking an experienced, dynamic, and professional Community health worker to join our team! The ideal candidate will have a strong background in community health workers and be adept at referring and recommending services to suit patients’ needs and requirements. The role's key focus is to provide short-term coordination to community members, connecting them with social, health, and wellness services they need access to.
Responsibilities
- Assist clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact or improvement it may have on their wellbeing.
- Help patients identify socio-economic issues that affect their health and develop health/social management plans and goals to assist them.
- Document all client encounters and contracts made on behalf of clients.
- Complete and submit monthly reports; maintain accurate electronic and hard copy client files, which include notes, the release of information, assessments, and other medical documents.
- Document activities, care and service plans, and outcomes or achieve goals achieved by the client in an effective manner.
- Educate clients on how to properly use the Emergency Room and provide information for viable alternatives.
- Coach patients in the management of their chronic health conditions and self-care.
- Assist patients in understanding care plans and instructions given by medical professionals. Motivate and encourage patients/clients to be active and engaged participants in their health and wellbeing.
- Assist clients in accessing health-related services, including obtaining a medical home, providing instruction on using the medical home appropriately, and overcoming barriers to obtaining needed medical care and /or social services.
Requirements
- Bachelor’s degree in healthcare administration, patient care, or related field
- Two years’ of hands-on experience in community health work
- Excellent written and verbal communication skills
- Good interpersonal and relationship-building skills
- Excellent problem-solving abilities
- Highly organized and able to prioritize one's own workload
- Strong customer service skills
- Computer proficient with proficiency in Microsoft Office
- Previous experience using EMR systems would be an advantage
- Remarkable analytical skills with strong attention to detail
