Job Brief
We are seeking an experienced and empathetic case manager to join our team. The successful candidate will be responsible for providing comprehensive case management services to clients, including assessment, planning, coordination, advocacy, and monitoring. Responsibilities

- Advocate for clients' rights and needs, and provide support and guidance throughout the case management process.
- Monitor clients' progress and adjust care plans as necessary.
- Maintain accurate and up-to-date client records, progress notes, and outcome data, in compliance with legal and ethical standards.
- Attend regular staff meetings, training sessions, and professional development opportunities to enhance knowledge and skills.
Requirements
- Bachelor's degree in social work, psychology, or related field
- Minimum of two years experience in case management, preferably in a healthcare or social services setting
- Strong interpersonal skills, including the ability to build trust and rapport with clients
- Excellent written and verbal communication skills, with the ability to effectively communicate complex ideas to clients, families, and healthcare providers
- Knowledge of community resources and support services
- Ability to multitask and prioritize competing demands
- Strong organizational skills and attention to detail
- Proficiency in computer skills and electronic record-keeping
- Ability to work independently and as part of a team
- Successful completion of a criminal background check and other pre-employment screenings
