Job brief
We are seeking a professional, knowledgeable, and personable Care Manager to join our team! The ideal candidate will have a strong background in care management and case management within the healthcare industry. The role's key focus is to provide overall management of care services offered by the facility and ensure set service levels are adhered to.
Responsibilities
- Collaborate with all service-providing departments to ascertain the patient levels, functionalities, and issues arising within the department.
- Create and develop effective management plans for the care being offered in each department.
- Work with department supervisors and managers to implement new management plans as required.
- Create and develop effective KPIs and service level agreements for each department offering care.
- Write, review and monitor risk assessments and update them when required.
- Assist department managers with arranging and executing required training for their team members.
- Ensure quality of care is high and within the expected levels set out by the business.
- Ensure care service departments are compliant in their work and use of patient information.
- Assist in hiring and training new care workers.
- Liaise with third-party vendors, such as suppliers, to ensure that required supplies are received when required.
- Manage vendor contracts.
- Write reports for executives on performance and improvements on a quarterly or monthly basis.
- Continually monitor services and make any necessary improvements where needed.
Requirements
- Master’s degree in Social Welfare, Social Services, Healthcare, or related field
- Minimum of four years’ experience in a care work setting
- Minimum of two years’ experience in a supervisory or management role in a care work setting
- Ability to identify and minimize risk, and write risk assessments
- Excellent communication skills, both written and verbal
- Ability to prioritize own workload and time effectively
- Demonstrated ability to lead change and improvement
- Highly proficient in Microsoft Office
- Confident in leading, training, and mentoring others
- Able to write effective reports and make presentations
