Job brief
As a trained health professional who helps manage a patient's care, the Care Coordinator monitors, and coordinates patients' treatment plans, connects them with healthcare providers, educates them about their condition, and evaluates their progress. This position requires organization, dedication, and compassion.
Responsibilities
- Guide patients and family members in discussions regarding their health conditions and/or concerns.
- Educate patients about their specific condition(s) and medication(s), providing detailed, personalized instructions.
- Create a care plan to focus on their personal medical needs.
- Collaborate with other healthcare providers and specialists to set up patient appointments and corresponding treatment plans.
- Regularly check on patients evaluating and documenting their progress.
- Assist the care team with developing and accessing health interventions.
- Keep apprised of the latest developments and best practices in healthcare.
- Complete compliance with governing laws, mandates, and regulations.
- Provide quality care treating patients with empathy and respect.
Requirements
- Bachelor’s degree in Social Work or a related field
- 3+ years experience in clinical practice and/or care coordination
- Excellent communication skills, both written and verbal
- Fluency in Microsoft Office and other related computer software
- Superb analytical thinking skills with the ability to multitask
- Exceptional customer service skills treating patients with compassion and empathy
- Ability to travel
